Informacje dotyczące przetwarzania danych osobowych (RODO)

W związku z przetwarzaniem danych osobowych informujemy:

1. Administratorem Pani/Pana danych osobowych jest Miejskie Centrum Kultury w Płońsku (dalej: „ADMINISTRATOR”), z siedzibą: ul. Płocka 50, 09-100 Płońsk tel. 23 662 27 02. Z Administratorem można się kontaktować pisemnie, za pomocą poczty tradycyjnej na adres: ul. Płocka 50, 09-100 Płońsk lub drogą e-mailową pod adresem: mck@mckplonsk.pl .
2. Administrator wyznaczył Inspektora Ochrony Danych, z którym można się skontaktować pod adresem mailowym: mck@mckplonsk.pl 
3. Pani/Pana dane osobowe są przetwarzane na podstawie Rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE (ogólne rozporządzenie o ochronie danych), tj. w oparciu o zgodę osoby, której dane dotyczą.
4. Przetwarzanie danych osobowych odbywa się na potrzeby kontaktu z Administratorem.
5. Przetwarzanie danych osobowych odbywa się na podstawie prawnie uzasadnionego interesu administratora lub strony trzeciej.
6. Dane osobowe nie pochodzą od stron trzecich.
7. Administrator nie zamierza przekazywać danych do państwa trzeciego lub organizacji międzynarodowej.
8. Administrator będzie przekazywał dane osobowe innym podmiotom, tylko na podstawie przepisów prawa.
9. Dane osobowe będą przetwarzane przez Administratora do zakończenia korespondencji bądź dłużej, w przypadku nawiązania współpracy.
10. Osoba, której dane dotyczą ma prawo do żądania od administratora dostępu do danych osobowych, ich sprostowania, usunięcia lub ograniczenia przetwarzania oraz o prawo do wniesienia sprzeciwu wobec przetwarzania, a także prawo do przenoszenia danych.
11. Skargę na działania Administratora można wnieść do Prezesa Urzędu Ochrony Danych Osobowych.
12. Podanie danych osobowych jest dobrowolne, jednakże ich niepodanie spowoduje brak możliwości kontaktu z Administratorem.
13. Administrator nie przewiduje zautomatyzowanego podejmowania decyzji.

 

In connection with the processing of personal data by the Municipal Cultural Center in Płońsk , we would like to inform you:

1. The administrator of your personal data is the Municipal Cultural Center

2. The Administrator has appointed a Data Protection Inspector, who can be contacted at the following e-mail address: iod@plonsk.pl or directly to the Administrator.
3. The Administrator processes contact information about the senders and addressees of e-mail correspondence, contained in the content of this correspondence, in order to:

• enabling e-mail contact with the administrator and contacting recipients,
• documenting arrangements made with clients, contractors and other persons,
• accepting letters, notifications and applications in electronic form, e.g. complaints, claims, other requests,
• protection against claims and investigation.

4. The legal basis for processing is our legitimate interest (Article 6(1)(f) of the GDPR), consisting in conducting correspondence addressed to us in connection with our activities.
5. The Administrator does not intend to transfer data to a third country or international organization.
6. The Administrator will transfer personal data to other entities only on the basis of legal provisions and on the basis of contracts entrusting the processing of personal data.
7. We store correspondence for the period necessary to properly and reliably achieve the goals referred to in section point 3 above, respectively for each purpose separately, unless the messages contain content important for pursuing claims or defending against claims, then we will store selected messages for up to 6 years, i.e. until the limitation period for claims, and in the event of initiating any court proceedings, also after that deadline. art. 6 section 1 letter f GDPR).
8. The data subject has the right to request from the administrator access to personal data, rectification, deletion or limitation of processing, as well as the right to object to the processing, as well as the right to transfer data.

the Administrator’s actions may be submitted to the President of the Personal Data Protection Office.

11. Providing personal data is voluntary, but required to undertake activities for the Municipal Cultural Center in Płońsk for the Studio of Documentation of the History of the City of Płońsk.
12. The Administrator does not provide for automated decision-making.
